Plymouth Community Homes (PCH) and health and social care provider Livewell Southwest have teamed up in a bid to improve the lives of people across the city.
PCH and Livewell Southwest commissioned the joint project to address the differences in local health inequalities and improve the quality of life for PCH residents, so they can live independently for longer in their own homes.
The project was created in 2021 when the boards of both organisations met and agreed to jointly fund a new project manager to run the initiative.
Rachael Fox was appointed as partnership project manager last summer, starting her role on 1 August 2022.
The partnership team has identified five key areas of health and wellbeing, including:
- Digital inclusion
- Resolving issues around admission to hospital and returning home
- Health improvement and health inequality in the community
- Improving air quality in our homes
- Developing a strategic plan for supported housing in the city
Rachael Fox, partnership project manager, said: “This is an exciting opportunity to build on our work with the health service.
“Our new partnership has the potential to make some positive differences to our residents’ lives, but also helpful changes for staff, and hopefully bring about a better way of working together, benefiting local people through more efficient services.”
Charlotte Edwards, head of Strategy, Policy, and Performance at PCH, said: “It is great to be working with Livewell Southwest on a range of projects that aim to improve the health and wellbeing outcomes of both PCH tenants and Livewell patients.
“We’re starting small but with the aim that Livewell Southwest would be able to scale up these initiatives and deliver them across their whole area of operation with other housing providers.”
